This order shall become effective when it has been signed by the City Administrator, City Engineer, Project Engineer, and
Contractor. All copies forwarded to Contractor for signature shall be returned to the City of Gilroy properly filled out. Upon
acceptance by the City, the Contractor's copy will be returned to him as his authority to proceed with the work.
Description of Work
Calstate to repair fractures to handball court corners as directed by COG. Per the request of the Gilroy Fire Marshal,
Calstate to replace the exit signs with Lithonia emergency LHQM Led exit lights with green lettering color. Fixture housing
color to be white. To remove the emergency exit sign within the temporary trailer and install into the utility storage room.
Install two new Chinese Pistache trees behind seatwall as per plan. Install additional landscaping as per Aitken &
Associates Landscape drawing dated 7- 27 -12. Repair sprinkler head, infill sanitation tank hole, soil prep and resod area
following the removal of the temporary trailer. Grind down tree stump and remove baserock at base of stairs at temporary
trailer. (a no cost time extension of 12 days will be added to the duration of the contract)
All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this
Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
By signing this Change Order, Contractor acknowledges and agrees that the adjustments to cost and time contained herein
are in full satisfaction and accord, and are accepted as payment in full, for any and all costs and expenses associated with
this Change Order, (the "Extra Work "), including but not limited to labor, materials, overhead and profit, delay, disruption,
loss of efficiency and any and all other direct and /or indirect costs or expenses associated with the Extra Work and hereby
waives any right to claim any further cost and time impacts at any time during and after completion of the Contract
associated with the Extra Work.

bills for work done under this Change Order shall
reference this Change Order No. 13.

As per our site meeting last Friday & per plan sheet L -1, we propose to
furnlsh & install the work described below:
1. Irrigation work per plan sheet L -1 & connect new valves to existing
controller in utility room.
2. Soil preparation, fine grade & sod the area between the new Youth
Center & the basketball courts and the area adjacent to the south
basket ball court. (approx. 2800 SF)
3. Re -seed all other areas in the field that are disturbed by our
installation of new PVC pipe & conduit
4. 30 day maintenance for new sod areas only
