This order shall become effective when it has been signed by the City Administrator, City Engineer.
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be relumed to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be relumed
to him as his authority to proceed with the work.
Description of Work
This Change Order provides for the following:
Location: Old Gilroy at Chestnut St.
. Scope of Credit for Work Deleted from Contract;
Sidewalk, Driveway, Curb &Gutter
Total Credit to City = $5,058.70
. Scope of Extra Work added to Contract;
Curb & Gutter
Total Additional Work = $8,853.00
Total Cost to City an Add of $3,794.30
All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this
Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
By signing this Change Order. Contractor acknowledges and agrees that the adjustments to cost and time contained herein
are in full satisfaction and accord, and are accepted as payment in full, for any and all costs and expenses associated with this
Change Order, (the "Extra Work"). including but not limited to labor, materials, overhead and profit, delay, disruption. loss
of efficiency and any and all other direct and/or indirect costs or expenses associated with the Extra Work and hereby waives
any right to claim any further cost and time impacts at any time during and after completion of the Contract associated with
the Extra Work.

Per agreed upon price. The cost of additional ramps is as follows:
1) To 'construct corners with double ramps at a cost of $6,800.00 per corner (two
ramps per corner). Total price per additional ramp of $3,400.00 per ramp.
Cost Breakdown for Ramps Below is as follows:
Saw = $ 520.00
Concrete = $3,521.10
Asphalt = $ 771.80
Base-Rock = $ 740.00
Debris = $ 690.00
Demo Crew = $2,502.15
Prep Crew = $1,904.10
Pour Crew = $3,240.02
Asphalt Crew = $1,270.00
Strip and Backfill = $1,160.00
Total Cost Breakdown = $18,767.05 + 15% OH = $2,815.06 + Profit = $1,182.11

Location and cost of additional ramps at contract locations:
1) Fourth @ Church = 1 additional ramp = $3,400.00
2) Fifth @ Rosanna = 2 additional ramps = $6,800.00
3) Fifth @ Church = Ramp per plan was already existing. JJ and Al agreed to cut out
landing surface of existing ramp and one additional and place truncated domes at
both ramps for the cost of one ramp. = $3,400.00
4) Fifth @ Hanna = 2 additional ramps = 6,800.00
Total Cost of Additional Ramps on Contract = $20,400.00
