Resolution 751 RESOLUTlON NO. 751 A RESOLUTION OF THE COUNCIL OF THE CITY OF GILROY ESTABLISHING A LIFE AND MEDICAL INSURANCE PROGRAM FOR THE EMPLOYEES OF THE CITY OF GILROY AND AUTHORIZING A CONTRACT. WHEREAS, it has been determined that the present medical insurance program with Cal ifornia Physicians Service does not provide the desired coverage for the City employees; and WHEREAS, an acceptable life and medical insurance program was developed by the Personnel Commission and an Employee Medical and Life Insurance Committee; and WHEREAS, the City Council on December 17, 1962 approved the proposed life and medical insurance program and authorized it to be advertized for bid, and WHEREAS, twenty-two bids on the proposed life and medical insurance program were received on January 21, 1963 and referred to the Personnel Commission for study and recommendation, and WHEREAS, the Personnel Commission studied the bids and proposals and did recommend to the City Counci I on March 18, 1963 that the proposal of the California-Western States Life Insurance Company did conform to the specifications and provided the lowest premium rate, and WHEREAS, the City Council did accept the Personnel Commissions' recommendation and did refer the insurance proposal to the city employees for their study and recommendation, and WHEREAS, the city employees did hold two meetings with representa- tives of the California-Western States Life Insurance Company and did elect to accept the original life and medical insurance proposal with the exception that a $100 deductible feature be substituted for the $50 deductible feature as requested in the original proposal, NOW, THEREFORE BE IT RESOLVED, that the City Administrator be authorized to contract with the Cal ifornia-Western States Life Insurance Company, at the prevailing rates for the proposed life and medical insurance program as follows: 75/ - 1 - I. Life Insurance, for each full time employee. a. $2,000.00 life insurance b. $2,000.00 accidental death and dismemberment insurance 2. Basic Medical Benefits, for each full time employee and his dependents. a. Hospital room and board benefit, per day, for 31 days, $20.00 b. Special hospital services 400.00 c. Ambulance benefit 25.00 d. Surgical benefits (schedule of operations) 500.00 e. Medical expense benefits (I) Hospital calls @ $4.00 per call 124.00 (2) Office calls @ $4.00 per call and Home calls @ $6.00 (for employees only) 200.00 f. Diagnostic x-ray and laboratory benefits 50.00 g. Additional accident expense benefits 300.00 h. Polio benefits 5,000.00 3. Supplemental Benefits, for each full time employee and his dependents. ao Major medical expense benefits b. Deductible, over and above amounts paid under "Basic Medical Benefits" (2). c. Maximum hospital daily room allowance considered to be covered "Covered Expenses" d. Percentage of "Covered Expenses" pa i d over "Basic Benefits" (2) and deductible (3-b) $10,000.00 100.00 35.00 80% 4. All other benefits and provisions as outlined in the bid specifications and those benefits and provisions which are considered standard by the California-Western States Life Insurance Company. BE IT FURTHER RESOLVED, that the City of Gi Iroy shall pay the premium costs for its employees and the employees shal I pay the premium costs of their dependents.
