This order shall become effective when it has been signed by the City Administrator,City Engineer, Project Engineer,and Contractor. All copies forwarded to Contractor for signature shall be returned to The City of Gilroy properly filled out. Upon acceptance by the City,the Contractor's copy will be returned to him as his authority to proceed with tine work. '. Description of Work This change order provides for One following: Item 1: Per the direction from the City,the contractor shall remove and reconstruct the stomndrain catch basin cover and lid,per the approved standards at the smith east comer of 9th Street and Church Street Intersection.All works associated with this additional scope shall be paid for under Force Account per Section 01035 Item 3.0 of the specification.The total cost for this work is$10,598.32 Item 2: Per the direction from the City,the contractor shall repair the pavement distresses on Santa Teresa Boulevard from Sunrise Drive to Mantelli Dr.The said distresses were not marked in the plan and required additional mobilization and different crew and equipment set-up.Therefore, all works associated with this additional scope shall be paid for under Force Account per Section 01035 Item 3.0 of the specification.The total cost for this work is$31,735.37 Item 3:(Bid Quantity Adjustment) - The purpose,of this item is to revise the contract amount to reflect the final installed quantities based on actual field conditions and deductions from the contract for items not performed and/or required to complete the work.Section 00310,Bid Schedule,shall be adjusted to include the following: Decrease in Contract Items of Work: Item No. Description Qty Unit Unit Cost Cost Ext.
