This order shall become effective when it has been signed by the City Administrator, City Engineer, Project Engineer, and
Contractor. All .copies forwarded to Contractor for signature shall be returned to the City of Gilroy properly filled out. Upon
acceptance by the City, the Contractor's copy will be returned to him as his authority to proceed with the work.
Descri� tion of Work
This change order is a deductive change order. Quantities of all the project bid items were tracked during construction. The
quantities used during construction for Bid Items 11, 12, 13, 14, 19, 20, 21, 22, 23, 29, 30, 31, 32, 33, 38, 43, 44, 49, 50, 51,
and 52 were less than the quantities described in the bid documents. Therefore, this change order reduces the contract value
to account for the difference between the actual quantities used during construction and the bid quantities. Details regarding
the quantities of these bid items are included in Attachment A.
All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this
Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
By signing this Change Order, Contractor acknowledges and agrees that the adjustments to cost and time contained herein
are in full satisfaction and accord, and are accepted as payment in full, for any and all costs and expenses associated with
this Change Order, (the "Extra Work"), including but not limited to labor, materials, overhead and profit, delay, disruption, loss
of efficiency and any and all other direct and/or indirect costs or expenses associated with the Extra Work and hereby waives
any right to claim any further cost and time impacts at any time during and after completion of the Contract associated with the
Extra Work.
Change in working time granted by this change order:
► All Extra Work authorized under this Change Order
must be billed separately from the original contract. All
bills for work done under this Change Order shall
reference this Change Order No. 2.

a.
In response to Submittal #4, Methacrylate Placement Plan, the contractor was directed to apply methacrylate at the rate of 90
SF/gallon as stated in 2015 Caltrans Standard Specification Section 60-3.03B(3). The methacrylate contract bid quantity was
estimated at a total of 446 gallons (bid items 11, 19, 29, 38, 43, and 49) for all six bridges. Per the Contract Plans, the total
surface area of all six bridges was 24,500 SF. Therefore, the methacrylate application rate for the Bid Quantity was 24,500 SF
/ 446 gallons = 55.0 SF/gallon. Since bid items 11, 19, 29, 38, 43 and 49 were not final pay items, the actual quantities were
tracked based on the revised application rate during construction of 90 SF/gallon. The total amount of methacrylate used for
all six bridges was 301 gallons, for a total deduction of 145 gallons. Therefore, the City is being credited for 145 gallons of
methacrylate at $45 per gallon = $6,525.
b.
Bid item #14, Thermoplastic Pavement Marking at the Church St Bridge, was only partially utilized. The contractor was
required to match the existing pavement markings per the Contract Specifications. Bid item #14 was bid at 20 SF. However,
there was only 11 SF of pavement markings at the Church St Bridge prior to construction. Therefore, the City is being credited
for 9 SF of pavement markings at $10 per SF = $90.
c.
Bid item #33, Thermoplastic Pavement Marking at the Forest St Bridge, was not utilized. The contractor was required to match
the existing pavement markings per the Contract Specifications. However, there were no pavement markings at the Forest St
Bridge prior to construction. Therefore, the City is being credited for 10 SF of pavement markings at $10 per SF = $100.
d.
Quantities of Thermoplastic Traffic Stripes (Bid Items 12, 13, 20, 21, 22, 23, 30, 31, 32, 44, 50, 51, and 52) were also tracked
during construction. The following is a summary of the total credit to the City for reduced quantities of thermoplastic traffic
stripes utilized during construction:

60-3.03B(1)(a) Summary
Section 60-3.03B includes specifications for treating bridge decks with ahigh-molecular-weight
methacrylate resin.
60-3.03B(1)(b) Definitions

60-3.03B(1)(c) Submittals
Submit a work plan for applying the methacrylate resin treatment. Include in the plan:
1. Schedule of work for the test area and for each bridge
2. Description of equipment for applying resin
3. Range of gel time and final cure time for resin
4. Description of absorbent material to be used
5. Description of equipment for applying and removing excess sand and absorbent material
6. Procedure for removing resin from the deck and equipment to be used
7. Procedure for storing and handling resin components and absorbent material
8. Procedure for disposing of excess resin and containers
Submit an SDS for each resin component and diatomaceous earth shipment before use.
Submit test samples of methacrylate resin components at least 15 days before use.
60-3.03B(1)(d) Quality Assurance
Complete a test area before starting deck treatment activities. Notify the Engineer at least 15 days before
treating the test area.
The test area must be:
1. At least 500 sq ft
2. Located within the project limits outside the traveled way at an authorized location
3. Constructed (1) using the same materials, equipment, and construction methods to be used in the
work and (2) under conditions similar to those anticipated when the work will be performed.
The completed test area must demonstrate (1) compliance with these specifications and (2) work will be
completed within the time allowed.
The Engineer performs friction testing of the treated test area under California Test 342. Allow 10 days
after completion of the test area for the Engineer to perform the test.
Do not perform deck treatment activities until the test area is authorized. The authorized test area is the
standard of comparison in determining the acceptability of treated deck surfaces.
The Engineer may perform testing under California Test 342 to verify the coefficient of friction of the
treated deck surfaces. The coefficient of friction of the treated surface must be at least 0.35 when tested
under California Test 342.

High -molecular -weight methacrylate resin consists of resin, promoter, and initiator.

Sand for the abrasive sand finish must be acommercial-quality, dry -blast sand. The size of the sand must
be such that not less than 95 percent passes the no. 8 sieve and not less than 95 percent is retained on
the no. 20 sieve when tested under California Test 205.
Absorbent material must be diatomaceous earth, abrasive blast dust, or an authorized substitute
recommended by the resin supplier.
60-3.036(3) Construction
Notify the Engineer at least 15 days before delivery of methacrylate resin components in containers larger
than 55 gallons to the job site.
The Engineer determines the exact methacrylate resin application rate at time of placement.
High -molecular -weight methacrylate resin applied by machine must be (1) combined in volumetric
streams of promoted resin to initiated resin by static in -line mixers and (2) applied without atomization.
You may apply resin manually. Mix at most 5 gallons of resin at a time.
The deck must be dry before applying resin. The concrete surface must be from 50 to 100 degrees F.
Relative humidity must be not more than 85 percent during the work shift.
Uniformly spread the resin. Completely cover surfaces to be treated and fill all cracks. Redistribute excess
resin using squeegees or brooms within 10 minutes of application. For textured or grooved deck surfaces,
remove excess resin from the texture indentations.
Apply the abrasive sand finish no sooner than 20 minutes after applying the resin. The sand application
rate must be at least 2 Ib/sq yd or until saturation as determined by the Engineer. Apply absorbent
material before opening the lane to traffic. Remove excess sand and absorbent material by vacuuming or
power sweeping.
Traffic or equipment is not allowed on the treated surface until you have verified that the following
conditions have been met and the Engineer has authorized the opening of the treated surface to traffic
and equipment:
1. Treated deck surface is tack free and not oily
2. Sand cover adheres and resists brushing by hand
3. Excess sand and absorbent material has been removed
4. No material will be tracked beyond the limits of treatment by traffic
Remove resin from the deck surface if the Engineer determines (1) the conditions listed above have not
been met and (2) the allowable lane closure time will be exceeded.

Covid 19 sign in sheet was signed by the workers. Hector Ortega check with workers on their health status regarding Covid-19 sign -in sheet. All
signs and cones placed as per each Bridge Site Traffic Plan. All workers had hard hat, reflective safety vest, steel toed shoes, safety glasses and
face masks as required by Covid-19. Operator of Shot blaster had required respirator while operating.
Inspections Made:
BRIDGE 37C-0320 LEWIS STREET:
ACC arrived and mobilized at 0700hrs but no road closures or work until 0800hrs. ACC closed one lane of traffic and utilized flaggers to control all
traffic. Once traffic was closed entire side was hand cleaned of all debris, then swept and vacuumed with street sweeper. Entire side was then shot
blasted and vacuumed utilizing Roadabrator. Entire Deck was hand swept and air cleaned using high pressure air. After deck (62'x38'=2356sf, say
2400sf) was thoroughly cleaned Inspector observed (Deck Temp of 71 F/Ambient 65F) approx. 27gals of Methacrylate hand mixed and uniformly
applied using 5gal buckets and squeegee at a approx 90sf rate as guided by Foreman for a uniform placement and to avoid puddling. Kwik-Bond Top
sand was hand applied to entire deck prior to Methacrylate completed curing. After approx 90min Solid -A -Sorb (Diatomaceous Earth Absorb)
absorbent placed using lawn fertilizer spreader to assure no puddling and to absorb added wax. After approx. 10min deck was vacuumed by
sweeper/vacuum one last time. Foreman and Inspector walked entire deck area checking for cure of Methacrylate and lane was reopened to traffic.
NOTE: Same process and placement application was completed on 2nd half of bridge and Inspected by Foreman and Inspector prior to reopening
Bridge up to two way traffic.

applied using 5gal buckets and squeegee at a approx 90sf rate as guided by Foreman for uniform placement and to avoid puddling. Kwik-Bond Top
sand was hand applied to entire deck prior to Methacrylate completed curing. After approx 90min Solid -A -Sorb (Diatomaceous Earth Absorb)

BRIDGE 37C-0339 MURRAY AVE:
ACC arrived and mobilized at 1200hrs but no road closures or work until 1215hrs. ACC closed one lane of traffic and utilized flaggers to control all
traffic. Once traffic was closed entire side was hand cleaned of all debris, swept and vacuumed with street sweeper. Entire side was then shot
blasted and vacuumed utilizing Roadabrator. Deck was again hand swept and air cleaned using high pressure air. After deck (106'x62=6572sf, say
6600sf) was thoroughly cleaned Inspector observed (Deck Temp of 127E/Ambient 86F) approx. 74gals of Methacrylate hand mixed and uniformly
applied using 5gal buckets and squeegee at a approx 90sf rate as guided by Foreman for uniform placement and to avoid puddling. Kwik-Bond Top
sand was hand applied to entire deck prior to Methacrylate completed curing. After approx 90min Solid -A -Sorb (Diatomaceous Earth Absorb)
absorbent placed using lawn fertilizer spreader to assure no puddling and to absorb added wax. After approx.10min deck was vacuumed by

and cones placed as per each Bridge Site Traffic Plan. All workers had hard hat, reflective safety vest, steel toed shoes, safety glasses and face

BRIDGE 37C-0363 CHURCH STREET:
ACC arrived and mobilized at 0700hrs but no road closures or work until 0800hrs. ACC closed one lane of traffic and utilized flaggers to control all
traffic. Once traffic was closed entire side was hand cleaned of all debris, then swept and vacuumed with street sweeper. Entire side was then shot
blasted and vacuumed utilizing Roadabrator. Entire Deck was hand swept and air cleaned using high pressure air. After deck (55'x98'=5390sf, say
5400sf) was thoroughly cleaned Inspector observed (Deck Temp of 76F/Ambient 86F) approx. 60gals of Methacrylate hand mixed and uniformly
applied using 5gal buckets and squeegee at a approx 90sf rate as guided by Foreman for a uniform placement and to avoid puddling.(NOTE:
Foreman and Inspector noticed that bridge deck was absorbing more than expected and deck required apprx 10ga1 extra of Methacrylate than what
was first applied. TOTAL 70gals.) Kwik-Bond Top sand was hand applied to entire deck prior to Methacrylate completed curing. After approx 90min
Solid -A -Sorb (Diatomaceous Earth Absorb) absorbent placed using lawn fertilizer spreader to assure no puddling and to absorb added wax. After
approx. 10min deck was vacuumed by sweeper/vacuum one last time. Foreman and Inspector walked entire deck area checking for cure of
Methacrylate and lane was reopened to traffic. NOTE: Same process and placement application was completed on 2nd half of bridge and Inspected
by Foreman and Inspector prior to reopening Bridge up to two way traffic.
NOTE: Inspector noticed prior to Methacrylate being applied that from BB to EB a noticeable longitudinal crack just West of Center line.
Measurement of the crack revealed that the crack had a maximum width of .080" or 3/32". Inspector then went to the bottom of the bridge and
