In response to your request, RBF Consulting is pleased to present this addendum to
provide the final design for the improvements of Santa Teresa Boulevard. Included in this
scope of work are the required modifications to the existing plans to provide for the join at
1St Street as a result of the plans being developed by Higgins & Associates and the
modifications to the plans as a result of the proposed undergrounding of utilities.
As a documentation item, the subconsultant contract with RGA provides for dry utility
design services "from" 1St Street to Day Road West. The addition of the 1St intersection
utility sheets plus bid schedule and specifications was considered additional services.
However, because of funding issues and coordination with the County, it is our
understanding that the services between Sunrise and Day Road West has been deleted
from the contract. As such, no additional fee will be requested for the 1St Street utility
plans.
Enclosed for your review are the following exhibits: Scope of Services, Exhibit "A" and
Proposed Compensation, Exhibit "B ".

TASK I FINAL DESIGN PLANS, SPECIFICATIONS AND ESTIMATES
TASK 1.01 IMPROVEMENT PLAN MODIFICATIONS
Consultant will modify previously prepared improvement plan sheets to join
to the new improvements at 1s' Street being prepared by Higgins and
Associates. Consultant will modify plan and profile to avoid relocation of
Verizon vaults. Consultant will modify improvement plans to accommodate
the new vaults being placed by PG& E. The following modifications are
anticipated:
• Insert Higgins and Associates improvement plans at 1St to show on
RBF plans as constructed base.
• Modify typical section sheets to reflect new join at 1St Street and
delete sections for construction on 1 St Street as no long applicable to
this project. Modify typical sections to reflect grading at vault. Provide
vault grading detail
• Delete RBF intersection detail plan at 1s' Street as no longer
applicable to this project.
• Modify demolition plan to reflect new base and to show median curb
removal required to construct Santa Teresa Blvd. north of 1St Street.
• Modify profiles Santa Teresa Blvd. north of 1St Street to join Higgins
plan and show median curb replacement removed under previous line
item for traffic control movements.
• Modify curb between Sta. 45 +00 and Sta. 45 +80 to avoid Verizon
manhole.
• Modify curb and median curb plan and profile between Sta. 58 +00
and Sta. 68 +00 to avoid two Verizon vaults.
• Adjust storm drain lateral A -3 at Sta. 22 +00, storm drain laterals E1
& E2 at sta. 66 +50 at storm drain lateral F1 at Sta 75 +70 as a result
of modifications to curb line to avoid Verizon vaults and join Higgins
plans.
• Modify soundwall plans in seven locations at a result of the installation
of the underground utility vaults.
• Modify soundwall plans between Sta. 65 +00 and Sta. 72 +00 as a
result of utility trench going behind curb line
• Adjust street light plans between sta. 58 +00 and sta. 68 +00 as a
result of new curb line.
• Add new joint trench and vaults to base drawings

• Delete metric street improvement plans and storm drain plans at 1"
Street as no longer applicable to this project.
• Modify 30sections as a result of changes in profile and curb lines and
grading around vaults.
• Modify sheet numbering, all sheets.
• Re calculate earthwork and soundwall quantities as a result in
changes in grading.
TASK 1.02 CONSTRUCTION STAGING / TRAFFIC CONTROL /STRIPING PLANS
Consultant will modify previously prepared traffic control plans and striping
plans to reflect new constructed base south of the 1St Street and to reflect
adjustment in curb line to avoid Verizon vaults between Sta. 58 +00 and Sta.
68 +00. It is anticipated that six traffic control sheets will require modification.
It is anticipated that three sign and striping sheets will require modification.
TASK 1.03 BOX CULVERT STRUCTURAL COVER
Consultant shall modify previously prepared plans to install eight foot wide
steel grate over the center of the box culvert structural cover on South Morey
Channel.
TASK 1.04 PG&E EASEMENT
Consultant shall provide easement in two locations for the installation of a
electrical vault on the northwest corner of Santa Teresa Blvd. and 1 St Street.
TASK 1.05 TRAFFIC SIGNAL MODIFICATION PLANS
Consultant will screen back traffic signal plan prepared by Higgins &
Associates at 1St Street and show loop replacements at the north side of
Santa Teresa Blvd. in a format acceptable by Caltrans and process as part
of the encroachment permit required by Caltrans for working through their
intersection. Provide for a modification of the Mantelli Drive Signal plan for
a power back up. Provide specification for the power back up on the Mantelli
Signal. Provide temporary street light plan for Mantelli Drive.
TASK 1.06 RETAINING WALL
Consultant will provide retaining wall design and grading plan at three
locations at the N.W. corner of Santa Teresa Blvd. and 1 St Street for the new
vaults. Plan to be developed separate from Santa Teresa Blvd. for inclusion
into the CRP plan set. Consultant to provide specifications and bid items.

The terms of the base contract agreement for the design of Santa Teresa
Boulevard provides for a 10% escalation clause for that portion of the
contract work remaining after December 31, 2001. Based upon the
December 2001 invoices, the remaining design work for the base contract
and addendum #1 is $47,676.00 As such the escalation amount for design
services is $4,768.00
TASK 1.08 ESCALATION CLAUSE, CONSTRUCTION SUPPORT SERVICES
The terms of the base contract agreement for the design of Santa Teresa
Boulevard provides for a 10% escalation clause for that portion of the
contract work remaining after December 31, 2001. Based upon the
December 2000 invoices, the remaining construction support services for the
base contract and addendum #1 is $23,430. As such the escalation amount
for the construction support services portion of the contract is $2,343
TASK 1.09 PROJECT MEETING TIME
Consultant will attend up to five (5) coordination meetings with the City and
other agencies during the final implementation of the Santa Teresa Blvd.
plans. Consultant will arrange meetings, provide discussion materials and
agendas and develop and distribute meeting notes.
Task 1.10 Profile Dry Utilities
Consultant will provide vertical profile for dry utilities based upon horizontal
layout prepared by RGA for Santa Teresa Blvd. Profiles will be at a scale of
1"=40' horizontal and 1"=4' vertical. Profile sheets will be double loaded.
Sections will be provided at locations of vaults. Profiles will show existing
ground, proposed finish street surface and subgrade along with top of
conduit elevations. Profiles will also show wet utility crossings. Sections for
the vaults will be provided in twelve locations. Consultant will provide
horizontal survey control sheet. It is anticipated that 6 sheets will required
for this task.

Consultant will provide additional design coordination and management
services to provide for separate utility contract. Consultant will provide:
• Review with RGA the constraints of the project and problem solve for
location of joint trench
• Incorporate joint trench designs, specifications and bid forms into
separate bid documents for utility contract
• Provide for additional government processing with the City and
County for separate utility contract.
EXCLUSIONS:
• Traffic Control Plans
• Any civil drawings associated with improvements
other than profiling and horizontal survey control plan
• Construction Staking or Verification

The total payment to CONSULTANT for all work, service, material, equipment and
incidentals necessary for performing and completing the Tasks set forth in this agreement,
shall be for the sum of $77,895.00
CITY shall compensate CONSULTANT in proportion to the services performed and
invoiced to the CITY as outlined below, subject to Article 4 of this agreement.

Blueprinting, reproduction, messenger service and other direct expenses will be charged
as additional costs plus a fifteen percent (15 %) handling fee. A Subconsultant
Management Fee of fifteen percent (15 %) has be added to the direct cost of any
subconsultant services to provide for the costs of administration, subconsultant
consultation and insurance.
