This order shall become effective when it has been signed by the City Administrator, Project
Engineer, and Consultant. Upon acceptance by the City, the Consultant's original will be
returned to him.
Description of Work:
Santa Teresa Boulevard Widening and Mantelli Drive Gap Closure project.
This Change Order extends the term of the agreement to February 8, 2006.

additional services for the final design for the improvements of Santa Teresa Boulevard. The
additional services include revising the previously prepared electrical lighting plans to meet IES
standards, and developing an irrigation loop system for Santa Teresa Boulevard.
Enclosed for your review are the following exhibits: Scope of Services, Exhibit "A" and Proposed
Compensation, Exhibit "B ".

EXHIBIT "A"
SCOPE OF WORK
SANTA TERESA BOULEVARD WIDENING
ADDENDUM #11
TASK I FINAL DESIGN PLANS, SPECIFICATIONS AND ESTIMATES
Task 1.01 Electrical Lighting Design
RBF will conduct photometric lighting evaluation (6 options) for Santa
Teresa Boulevard to meet IES standards, revise the current plans tc
implement the criteria and calculate new wiring diagrams for the plans.
RBF will provide revise cost estimate.
Task 1.02 Irrigation Plans
RBF will revise the previously completed irrigation plans to provide for an
irrigation loop system with points of connections at Mantelli Drive and
Longmeadow Drive. RBF will delete the current point of connection at
First Street and delete the irrigation and landscaping on the bank property.
RBF will provide for a service truck turn out at the Longmeadow point on
of connection. RBF will provide a revised cost estimate for the irrigation
plans. RBF will revise specifications for repair of damaged landscaping
and irrigation system and bank.

The total payment to CONSULTANT for all work, service, material, equipment and
incidentals necessary for performing and completing the Tasks set forth in this
agreement, shall be for the sum of $9,700.00
CITY shall compensate CONSULTANT in proportion to the services performed and
invoiced to the CITY as outlined below, subject to Article 4 of this agreement.
