HomeMy WebLinkAboutDokken Engineering - 2004 Agreement - Amendment No. 07 AMENDMENT TO THE AGREEMENT FOR SERVICES BETWEEN CITY OF GILROY AND DOKKEN ENGINEERING FOR CAMINO ARROYO BRIDGE CONSTRUCTION AND GAP CLOSURE AND THE FARRELL ROAD BRIDGE WIDENING DATED AUGUST 16.2004 AMENDMENT NO.7 This Amendment shall become effective when it has been signed by the City Administrator, Project Manager, and Consultant. All copies forwarded to Consultant for signature shall be returned to the City of Gilroy properly filled out. Upon acceptance by the City, the Consultant's copy will be returned to him as his authority to proceed with the work. Preparation of plans, specs and estimates (PS&E) for the Sixth & Eigleberry curb extensions and various sidewalk repairs on Sixth Street and associated construction support - see attached scope of work and fee. This Amendment extends the term of the Agreement for Services between the City of Gilroy and Dokken Enaineerina, dated Auaust 16. 2006 to December 31. 2012. All requirements of the original Agreement Documents shall apply to the above work except as specifically modified by this Amendment. The contract time shall not extend unless expressly provided for in this Amendment. The proposed scope of services is described in the attached Exhibit A, Scope of Services, Contract Amendment #7, by task and subtasks. The hours associated with each subtask are broken down by personnel on Exhibit B - Estimated Labor Hours and Cost, Sixth Street Improvements, Contract Amendment #7. This work will be billed on a time and materials basis with a not-to-exceed amount as shown on Exhibit B. The existing contract amount, remaining unspent budget and proposed budget for the Amendment #7 tasks are shown in the following table. The total compensation for this amendment shall not exceed $48,138. EXHIBIT "A" SCOPE OF SERVICES TASK 3 - CONSTRUCTION SUPPORT FOR RAILING Task 3.1 Supplemental Inspection Dokken Engineering (Dokken) will provide inspection of the Sixth Street Barrier removal and finishing of the barrier cap. Dokken intends to provide inspection for four days to supplement the City management and inspection of the construction project. a. Inspection of the barrier removal system and marked sawcut removal line b. Barrier removal operation (for one day only) c. Barrier concrete cap operation (for one day only) d. Installation of the welded iron railing (for one day only) TASK 4 - EIGLEBERRY AND SIXTH STREET CURB EXTENSIONS For this task, Dokken will be incorporating curb extension designs prepared by Harris and Associates (Harris) into the Sixth Street Barrier Project for ease of City contracting. This work was prepared by Harris under a prior design contract and accepted by the City. Dokken is subcontracting to Harris for the purpose of packaging the curb extension design only. Dokken is not responsible for the assumptions, accuracy or quality of the plans prepared by Harris. Dokken's responsibility is to incorporate the curb extension plans prepared by Harris into the Sixth Street Barrier Project and supplement their design with traffic handling plans and street lighting modifications. Task 4.1 Prepare Draft Plans (Base + Add Alternate) Harris will prepare draft plans for curb extensions, pedestrian ramps, and storm drains at the Sixth Street and Eigleberry Street Intersection. The plans will differentiate between elements included in the base bid, and those included in the add-alternate option. Improvements. Storm Drains None None EXHIBIT A, Scope of Services ~~?I~~'~~ <. Dokken will prepare draft plans for traffic handling and street lighting at the Sixth Street and Eigleberry Street Intersection. The plans will differentiate between elements included in the base bid, and those included in the add-alternate option. No surveying services are included in this proposal. Harris has been advised by the City to assume the curb extensions on the east corners of Sixth Street and Eigleberry Street have been constructed to the grades shown on the plans. Task 4.2 Prepare Specifications and Quantities Harris will prepare special provisions and quantities for the curb extensions to be included with the bid of the Sixth Street Barrier Modification Project. Dokken will prepare special provisions for the street lights and be responsible for incorporating the work by Harris into the bid documents. The quantities will be added to the bid schedule in items designated to the Eigleberry and Sixth Street Intersection. There will be sections in the bid schedule for the base items and the add-alternative items. The special provisions will be based on 2006 Caltrans Standard Specifications with modifications for City preferences and standards. Task 4.3 Revise Plans (Base + Add Alternate) The draft plans will be revised to address comments from the City and the 100% design detailing. The final plans will be incorporated In the Sixth Street Barrier Modification Project. The plans will clearly identify that work covered under the add-alternate bid schedule. Task 4.4 Construction Support Dokken will provide assistance during bidding including answering questions from bidders and providing interpretation of the construction documents. After construction begins, we will provide ongoing consultation and interpretation of contract documents, as requested. Written responses will be provided to all requests for information (RFI's) and supplemental or revised drawings may be issued to provide clarity to the Resident Engineer and contractor. Upon completion of construction, Dokken will prepare the as-built plans or record drawings based on the information provided by the inspector or resident engineer. TASK 5 - SIDEWALK REPAIR ON SIXTH STREET This task includes the preparation of plans and specifications for the replacement of a sidewalk section west of Monterey Street on the south side of Sixth Street. The project includes replacing a failed section of sidewalk in between existing glass panel sidewalk sections without disturbing or removing the glass-block panels. It is not proposed that the sidewalk be replaced in its entirety to match the Sixth Street corridor because of the glass- block sidewalk sections. This proposal assumes the sidewalk will be bid as part of the Sixth Street Modification Project. Task 5.1 Prepare Draft and Final Plans Dokken will prepare a draft plan for the sidewalk removal, under sidewalk excavation of failing subgrade material, slurry backfill, and sidewalk replacement. The plan includes leaving the existing glass-block sidewalk panels in place. Dokken anticipates the placement of slurry backfill under the broken sidewalk section, between the existing basement buttress walls. We have looked at the buttress walls inside the basement and they appear to be in good condition. However, we have no way to know exactly what conditions exist behind the buttress walls, or if any reinforcement exists in them. Therefore, it may be necessary to revise the planned construction after removal of the sidewalk and some subgrade material. The City understands that Dokken has no way of knowing what conditions will be found once the sidewalk is removed. Because of this, it is fully anticipated that a construction change order will be necessary to adjust the work once the under sidewalk conditions can be observed. The draft plan will be revised to address comments from the City. The final plan will be incorporated in the Sixth Street Barrier Modification Project. EXHIBIT A, Scope of Services ~~?,~~'~~ \. Written responses will be provided to all requests for Information (RFI's) and supplemental or revised drawings may be issued in the event that custom reinforced concrete corbels or sections are necessary to support the new sidewalk section.
