CHANGE ORDER NO.2
To contract for: Parking lot property line survey/staking, additional parking lot striping, bike rack revision, additional
planter brick pavers (from 4' wide strip to 6' wide), sawcut for driveway, utility and paving w/in Sixth Street.
Contractor: Don Chapin Company Contract Date: July 13th, 2010
This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Description of Work
This Change Order provides for the following:
(I) Survery work to identify City/RR right-of-way line to ensure proposed improvements are to be located outside
RR and within City right-of-way (2) Additional striping to parking lot to for additional stalls (3) Revisions from
embedded bike racks to surface mounted bike racks (4) Add additional area of brick pavers along Sixth Street from
4' wide planter strip to 6' wide planter strip to match planter strip width of Monterey Road and to allow for proper
alignment of street trees and street lights (5) Sawcut driveway at rollup door, south side of sixth just east of
Monterey (6) Catch Basin on North West Comer of Railroad Street (7) Remove 4' of additional street surface and
repave to remove trench line (8) New service to 12" (Quantity 5) & Reconnect to 12" (Quantity I)
This Change Order is being issued directly to Don Chapin as part of the original contract.
See attached Change Order request for items from Don Chapin.
All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this
Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
By signing this Change Order, Contractor acknowledges and agrees that the adjustments to cost and time contained herein are
in full satisfaction and accord, and are accepted as payment in full, for any and all costs and expenses associated with this
Change Order, (the "Extra Work"), including but not limited to labor, materials, overhead and profit, delay, disruption, loss of
efficiency and any and all other direct and/or indirect costs or expenses associated with the Extra Work and hereby waives any
right to claim any further cost and time impacts at any time during and after completion of the Contract associated with the
Extra Work.
