HC Ramp on Eigleberry by Vets Building & Patching at Milias Restaurant
Contractor: Don Chapin Company Contract Date: July 13th, 2010
This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Description of Work
This Change Order provides for the following:
Eigleberry Parking Lot
Old City Hall Parking Lot Modifications
SW Corner of Sixh and Monterey
HC Ramp on Eigleberryby Vets Building
Patching at Milias Restaurant
All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this
Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
By signing this Change Order, Contractor acknowledges and agrees that the adjustments to cost and time contained herein
are in full satisfaction and accord, and are accepted as payment in full, for any and all costs and expenses associated with this
Change Order, (the "Extra Work"), including but not limited to labor, materials, overhead and profit, delay, disruption, loss
of efficiency and any and all other direct and/or indirect costs or expenses associated with the Extra Work and hereby waives
any right to claim any further cost and time impacts at any time during and after completion of the Contract associated with
the Extra Work.

Our proposal includes the following items based on jobsite investigation. No soils report or engineering
documentation was provided at the time of this proposal, therefore we cannot warrant against settlement, heaving,
cracking or adverse drainage characteristics. The following proposal is to re-construct a pavement and concrete areas
at the location referenced above. Due to the nature of the loading in this particular area and the lack of subsurface
infonnation or engineering we cannot ensure the thickness proposed will be correct for the intended use (we can
provide a qu.ote for a different design at your request).
Base Bid
. Remove and replace approximately 54 Tons of existing asphalt.
. Remove 1 EA driveway entry and replace with city standard sidewalk curb and gutter.
. Remove a section of sidewalk and replace with commercial driveway approach.
. Includes deeplift asphalt patching necessary for formwork relief at concrete repairs.
. Sighage and striping included.
Lump Sum: $23,350.00

Pricing Subject to conditions per Attacment "B"
Fees (railroad fla22ing, encroachment or other), or Permits are excluded.
Any Staking, Compaction testing, or Engineering is excluded.
Any additional signage is excluded.

Jim, cost for parking lot modifications is as follows:
1. Lower existing catch basin and add H-20 traffic rated ADA grate.
2. Remove dirt and gradr to drain area along RR tracks.
3. Misc. conform grinding as quantified on attached email
4. Asphalt Paving +/- 2100 sf depth varies 2"-3"
5. Single application of "OVERCOAT" sealer +/- 5300sf
6. Re stripe and add ADA signage as indicated on attached email
