Contract Date: 7/16/2007 This order shall become effective when it has been signed by the City Administrator, City Engineer, Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned to him as his authority to proceed with the work. Description of Work Remove soil and replace with decomposed granite per attached extra work invoice dated 4/1 0/09. Patch, remove/replace and add sidewalk and curb & gutter at various locations per attached extra work invoice dated 1/6/09. Additional excavation to increase structural section from 12" to 14" at the Sixth Street/Rogers Lane intersection per attached extra work invoice dated 10/9/08. Additional surveying at the Sixth Street/Railroad Street intersection per attached extra work invoice dated 10/9/08. Revisions to pilaster footings per attached extra work invoice dated 1/28/08. Additional work related to fire hydrants and gate valves per attached extra work invoice dated 11/7/07. All requirements of the original Contract Documents shall apply to the above work except as specifically modified by this Change Order. The contract time shall not extend unless expressly provided for in this Change Order.
