This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
the City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be
returned to him as his authority to proceed with the work.
Description of Work:
Slip-Out Repair: Excavate 4 feet wide by 2 feet deep by 40
feet long keyway along toe of slope. Remove loose material
from the slip-out area. Place and compact imported engineer
fill in the slip-out area. Fine grade slope to match adjacent
slope. Hand broadcast seed and cover with straw. (see
attached back-up)
Total
$18,280.00
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Dear Mr. Goodhue.
Please find attached Granite Construction Company's Scope of Work, Bid Proposal,
and Special Conditions for the above referenced project. This proposal has been
prepared in accordance with direction given during the site investigation on Monday.
July 12, 2004.

broadcast seed and cover with straw. Install 50 If of trench drain to handle seepage
observed on Tuesday, July 20, 2004.
Vibratory compaction equipment will be used in the slip-out repair. Granite Construction
will not be responsible for repairing other slip-out areas that could possibly develop
during and/or after construction. Additionally, Granite Construction will not be
responsible for damage to the concrete structure during and/or after construction
(except through contractor negligence).
