Contract Date: 5/17/04
This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
the City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be
returned to him as his authority to proceed with the work.
Description of Work:
Color concrete, brick pattern, near Welburn Bridge (see attached back-up).
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Re: Cost Proposal for Color Concrete - Brick Pattern Near Welburn Bridge
Dear Mr. Goodhue,
Seope of Work:
This Cost Proposal includes the following: Remove approximately 330 sf of sand material at present
site location, and replace with 4" of Color Concrete with the brick pattern. This work needs to be
scheduled during our concrete operations at Oak Commons or Santa Teresa Boulevard.
General Exclusions:
1. Over-excavation and/or recompaction due to unsuitable ground conditions; this will be paid as
extra work.
2. Handling. and/or removing of any hazardous materials.
