Contract Date: 5/17/04
This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
the City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be
returned to him as his authority to proceed with the work.
Description of Work:
Replace project sign and incorporate sign revisions per attached Letters No.8 & 47.
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Construction Sign Changes
The signs shown on the Phase 2 Traffic Control Plans on northbound Santa Teresa south of First
Street are to be installed for Phase 1. In addition, a C40 sign is to be added to NB Santa Teresa
at Station 6+00. Finally, the speed limit shown on the C17 signs is to be changed to 35 MPH.
Please provide an itemized lump sum cost proposal for the additional signs.

RE: Santa Teresa Boulevard Widening Project No. 04-CDD-128
Sign Changes
Reference is made to Letter No.4 dated July 16,2004 regarding the project sign and
Letter No.8 dated July 23, 2004 regarding construction sign changes.
As discussed in our weekly meeting, the existing project sign is to b~ replaced with a
7' x 11' sign with 6" x 6" posts. The lump sum amount of $1776 will be paid to
cover all costs associated with the sign changes indicated in Letter No.8 as well as
the larger project sign as indicated above.
