Contract Date: 5/17/04
This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
the City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be
returned to him as his authority to proceed with the work.
Description of Work:
Remove existing soundwall and perform grading in preparation for soundwall extension at the northwest
comer of Mantelli Drive/Santa Teresa Boulevard (per attached Letter No. 27 and Change Order Request
No. 14).
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Extend Sound wall at NW Corner Mantelli
The new soundwall at the northwest comer of Mantelli Drive/Santa Teresa
Boulevard is to be extended to Station 9+11.71. The existing soundwall is to be
removed east of this Station and the new wall will join the existing wall at this point.
Please provide a cost proposal for removal of the existing wall.
Revised drawings providing the layout and dimensions of the new soundwall will be
issued shortly. We anticipate that the extended soundwall will be paid under Bid
Item 107.
