This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies fonvarded to Contractor for signature shall be returned to
The City of Gilroy properly filled ont. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Descriotion of Work
Implement changes to traffic control plan per attached Change Order Request No. lOA.
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Traffic Control Changes
Reference is made to Letter No. 19 dated August 26, 2004 regarding striping
modifications at Santa Teresa at First Street. We have determined that these striping
modifications are not necessary at this time. However, we have since identified
several safety concerns that must be addressed immediately. You are directed to
implement the attached changes as soon as possible. Additional costs for performing
this work will be paid at force account.

TRAFFIC CONTROL CHANGES
1. Relocate stop ahead sign for the westbound to northbound right
turn from First to northbound Santa Teresa further east on First
Street.
2. Furnish and install additional stop sign on the right and a stop
legend for the westbound to northbound right turn from First to
northbound Santa Teresa.
3. Extend center line channelizers on the north leg of Santa Teresa
closer to the intersection.
4. Add a straight arrow for the northbound Santa Teresa lane on the
north side of the First Street intersection.
5. The skip line from the eastbound 152 left turn to northbound
Santa Teresa comes off of the incorrect center line. This should be
corrected and reflective pavement markers added to both skip lines
for the left turns. Existing skip lines do not provide 30'
clearance as shown on plans.
6. Add right lane closed signage for the eastbound approaches at
Welburn and Longrneadow. Verify at all locations corning into the
project that a minimum of three warning signs are visible. (Lane
closure signs are acceptable in addition to the road construction
ahead and road construction signs.)
7. Low shoulder signs shall be installed for all areas that have a
drop. (See the Caltrans Traffic Manual for minimum drop requiring
signage.)
8. The barricades require yellow flashers. (The northerly and
middle section of the project had no or extremely few flashers.)
9. The delineators require a tighter spacing.

10. Additional reduced speed signs are required. A reduced speed

11. Additional glue down channelizers need to be added to the
painted median north of the Mantelli intersection to prevent
motorists from entering this median area (at speed) to get to the
left turn lane. If the left turn lane can safely be extended that
should be the first choice. If there is not adequate room the
channelizers need to be placed.

Additional Traffic Control Changes
In addition to the work specified in Letter No. 22 dated September 2,2004, the right
turn lane from westbound First Street to northbound Santa Teresa is to be striped per
the attached detail. Additional costs for performing this work will be paid at force
account.
