This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work
Description of Work
. Upgrade #5 interconnect boxes shown on Drawing No. SIC-2 to #6 boxes.
. Install extensions on all interconnect boxes and install #7E boxes for all Quad Duct street crossings per
attached letter No. 64.
. Install Quad Duct and street light conduit side-by-side with 30" minimum cover.
. The unit price for Bid Item No. 147 is revised from $4001EA to $6801EA All #6E and #7E interconnect boxes
will be paid under Bid Item No. 147 at the revised unit price.
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Quad Duct Pull Box Clarifications
The pull boxes for the Quad Duct are to be installed as follows:
Pull boxes for the Quad Duct street crossings shall be #7E per RFI 17. All other
Quad Duct pull boxes shall be #6E instead of #6 and shall be installed at 200'
maximum spacing. All Quad Duct, including street crossings, shall enter and exit on
the short side (north and south end) of the pull box.
