This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Description of Work
Adjust unit price for Bid Item No. 99, Tapping Sleeve & Valve, due to decreased quantity per attached Change
Order Request No. 37.
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

Charlie,
The ol'iginal contract for Bid Item No. 99 Tapping Sleeve and Valve called for the installation of six 24" x 6"
tapping sleeves. Only two tapping sleeves were installed per the direction of the City of Gilroy. Due to the
decreased quantity experienced for Bid Item No. 99 Granite Construction Company would like to adjust the
contract unit Pl'ice via force account per Section 1035-4.0 of the Contl'ad Specifications.
Please find the attached force account analysis for the work involved with installing the two tapping sleeves at
station 44+20 and station 68+00.
