This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Description of Work
Replace existing conduit and pullboxes at Welburn Avenue, Mantelli Drive and Longmeadow Drive per attached
Change Order Request No. 69.
All requirements of the original Contract Documents shall apply to the above work except as specifically
modified by this Change Order. The contract time shall not extend unless expressly provided for in this
Change Order.

The below prices are for the replacement of existing conduit and boxes. The locations in the
field did not correspond with locations on the plans. The existing conduit and boxes were
located in the new street and therefore had to be replaced. After discussions with the city
it was concluded that it would be better to replace the OLC conduit rather than use one of
the quad-duct conduits. Due to time constraints GESI has installed the boxes and conduit.
DLC work on the south east side of Mantelli Dr.
200 LF 1 1/2 Sch. 40 OLC conduit @ $7.00 = $1,400.00
2 EA #5 Concrete Pull Boxes and Lids @ $500.00 = $1,000.00
DLC work on the south east side of Welburn Ave.
