This order shall become effective when it has been signed by the City Administrator, City Engineer,
Project Engineer, and Contractor. All copies forwarded to Contractor for signature shall be returned to
The City of Gilroy properly filled out. Upoo acceptance by the City, the Contractor's copy will be returned
to him as his authority to proceed with the work.
Description of Work
Remove and replace channel driveways per attached Letter No. 003 and Change Order Request No, 87.
All requirements of the original Contract Docwnents shall apply to the above work except as specifically modified by
this Change Order. The contract time shall not extend unless expressly provided for in this Change Order.

Commercial Driveway Approaches at the South and North Morey Channels
After reviewing the grading details on sheet 11 of 113 (DT6), it has been determined to eliminate and
remove the commercial driveway approaches (Item 33 of the Construction notes) at the South and North
Morey Channels. The removal of the driveway approaches will eliminate the 8.33% slope in the sidewalk
and eliminate the depression in the curb and gutter.
The curb and gutter and sidewalk are to be per City of Gilroy standard detail STR-16. All other items
including the removable guard rail, gates, etc., are to be per plan. Listed below are the fallowing
locations:
1). Sta. (54+50.50) South Morey Channel
2). Sta. (55+14.50) South Morey Channel
3). Sta. (72+28.50) North Morey Channel
4). Sta. (72+91.00) North Morey Channel
Cost for rework resulting from this change will be paid on a time and material basis.
