The services include 1) conduct field surveys and finalize geometrics for Santa Teresa Boulevard between Day Road East and Day Road West 2) prepare dry utility relocation plans for Santa Teresa Boulevard. An additional budget for reimburseable expense has been included in this addendum. This reimburseable expense budget is for the entire Santa Teresa Boulevard project with the anticipated processing for the Mantelli Drive Gap Closure Project and the remaining efforts on Santa Teresa Boulevard. Topographic surveys will extend 300' east on Day Road East and 500' west on Day Road West. Locations of surface features will be collected including utility structures and other manmade features. Consultant will prepare a topographic map file in AutoCAD, Version 14 format derived from the field survey. Mapping will be a scale of 1" = 40'. A electronic file will be transmitted to the City upon completion of the mapping. TASK 1.2 RIGHT OF WAY CALCULATIONS Consultant will research, assemble existing right of way maps and plats and calculate in the existing right of way of Santa Teresa Boulevard from Day Road East and 500' north of Day Road West and Day Road East for 400' and Day Road West for 600' feet. Specifically excluded from this scope of work is the preparation of any legal descriptions and exhibits for any new right of way that may need to be acquired to complete the utility relocation requirements. TASK 1.3 GEOMETRIC APPROVAL DRAWINGS Consultant will prepare geometric approval drawings for Santa Teresa Boulevard from Day Road East to 500' north of Day Road West. The intersections of Day Road East and Day Road West with Santa Teresa Boulevard will be reconfigured to provide for a ninety degree intersections. The intersections will be designed for a 35 mph approach design speed utilizing a 600' radius approach curves. TASK 1.4 BASE DRAWINGS Consultant will prepare double loaded base drawings at a scale of 1" = 40' from 1000' south of Sunrise Drive to 500' north of Day Road West. Base drawing will incorporate approved geometric drawing showing ultimate improvements and topographic information. Consultant will notify dry utilities and incorporate existing utilities into base drawings. Base drawings will be transmitted to RGA for their use in preparing dry utility relocation drawings. RBF Consulting July 2, 2001 Contract Agreement for JN 35- 100063.007 Page 3 of 5 TASK 2.0 UNDERGROUNDING OF DRY UTILITIES TASK 2.1 UTILITY COMPOSITE DESIGN Consultantwill prepare utility composite (P.G.& E., Telephone, CATV) design and related consulting services for this project. The design shall consist of a composite for approximately 91 00' along Santa Teresa Boulevard from 1s` Street to Day Road West; a new joint trench composite for Mantelli Drive from Santa Teresa Blvd. to Zinnia, and an interim design for the intersection improvements at Santa Teresa Boulevard and Mantelli Drive. Consultant shall provide the following services: • Expedite and coordinate project design with utility companies. • Perform field analysis and review • Conduct eight (8) design team meetings • Conduct utility engineering meetings. • Complete composite construction drawings (65% & 100 %) • Perform value engineering • Prepare Form "B" • Prepare bid package and specifications and unit price bid form • Review and analyze utility contracts and fees. • Schedule and attend pre- construction meeting and job start up. Consultant services to be provided by RGA Utility Consultants, Inc. TASK 2.2 DRY UTILITY DESIGN COORDINATION Consultant will provide design coordination and management services to facilitate the activities of RGA. Consultant shall: • Review with RGA the constraints of the project and problem solve for location of joint trench • Attend utility meetings as required to develop joint trench design. (Anticipate two meetings) • Incorporate joint trench designs, specifications and bid forms into bid documents being prepared by the consultant. • Services apply to both the Mantelli Drive Gap Closure Project and the Santa Teresa Boulevard Widening Project. • Provide for additional government processing with the City, County and Water District as a result of the joint trench. TASK 2.3 MEETING ATTENDANCE Consultant will attend up to four (4) coordination meetings with the City and other agencies during the implementation of this additional work. Consultant will arrange meetings, provide discussion materials and agendas and develop and distribute meeting notes. Drainage Studies 2. Geotechnical Reports 3. Final Street, Drainage, & Traffic Improvement Plans Between 1000' south of Sunrise Drive to Day Road West 4. Extension of EIR to Day Road West 5. Construction staking, inspection, administration, construction quantity calculations and management. 6. Legal Descriptions and Exhibits 7. Any item not specifically identified above. RBF Consulting July 2, 2001 Contract Agreement for JN 35- 100063.007 Page 5 of 5 EXHIBIT "B" "PAYMENT SCHEDULE" SANTA TERESA BOULEVARD WIDENING ADDENDUM #6 The total payment to CONSULTANT for all work, service, material, equipment and incidentals necessary for performing and completing the Tasks set forth in this agreement, shall be for the sum of $98,538.00 plus a budget of $10,000.00 for reimburseable expenses. CITY shall compensate CONSULTANT in proportion to the services performed and invoiced to the CITY as outlined below, subject to Article 4 of this agreement. A Subconsultant Management Fee of fifteen percent (15 %) has be added to the direct cost of any subconsultant services to provide for the costs of administration, subconsultant consultation and insurance. No escalation of fees for remaining work shall apply in the year 2002.
