Contractor: Perma-Green Hydroseedine; Ine Contract Date: 12/18/03
This order shall become effective when the City Administrator, City Engineer, Project Engineer, and Contractor have signed it. All
copies forwarded to Contractor for signature shall be returned to the City of Gilroy properly filled out. Upon acceptance by the City,
the Contractor's copy will be returned to him as his authority to proceed with the work.
DESCRIPTION OF WORK:
Make the following revisions to the Safer Route to School Phase 1 plans supplied and specified per CCO No.7
& 14. This work will be compensated for at the following agreed to lump sum prices:

Revise keyway detail per CCO No. 14; Remove and off-haul rock slope protection from fill slope - $7,945.00
Additional grading due to incorrect topography elevations on plans for station 3+50 to 5+50
- $5,334.00
C,. Extend 36" CMP, pour concrete collar and remove unsuitable material at station 6+00
