All copies forwarded to Contractor for signature shall be returned to the City of Gilroy properly filled out. Upon acceptance by the City, the Contractor's copy will be returned to him as his authority to proceed with the work. DESCRIPTION OF WORK: Provide all labor, equipment, surveys, and materials to revise work specified per Contract Change Order 7 (Drawings C-l through C-6, dated and issued November 11, 2002) to the work specified per the phase one limits on revised plan sheets C-l through C-7 (dated 7/14/03), L-2 through L-6 (dated 7/09/03), including the trail portion on the south east side of Santa Teresa Bridge (connects bridge sidewalk to existing school trail) and keyway detail dated 2/11103. All work shall be in conformance with required permits. No work shall occur within 50 feet of an occupied swallows nest. The additional specified work will be done at the agreed to lump sum prices of: Grading using onsite fill material 18' Double wide swing gate 1800sf of CLII AB for SCVWD access Increased AC section to 4" at (Station 5+50 to 6+00) AC Paving and CL II AB at South end of bridge AC Paving & Base Signs and striping Keyway Work (Station 6+00 to 7+00) Additional 15" Storm Drain Inlet 8 Collapsible Bollards (per note 5 on page C-5) Sleeves and Surveys Hydroseeding, Planting and Revegetation Total This Change Order $18,500.00 $4,360.00 $3,600.00 $600.00 $7,200.00 $13,000.00 $2,540.00 $39,000.00 $5,500.00 $1,200.00 $12,000.00 $8,500.00 $116,000.00 Section 22, "Wage Rates" and paragraph No.1 of 4.11.1, "Prevailing Wage" of the project specifications is amended to the following: If there is a difference between the minimum wage rates predetermined by the United States Secretary of Labor and the general prevailing wage rates determined by the Director of the California Department of Industrial Relations for similar classifications of labor, the Contractor and subcontractors shall pay not less than the higher wage rate. The City will not accept lower State wage rates not specifically included in the Federal minimum wage determinations. This includes "helper" (or other 1 classifications based on hours of experience) or any other classification not appearing in the Federal wage determinations. Where Federal wage determinations do not contain the State wage rate determination otherwise available for use by the Contractor and subcontractors, the Contractor and subcontractors shall pay not less than the Federal minimum wage rate which most closely approximates the duties of the employees in question. This change order adds 260 days to the time allowed for completion specified in section 00810-2 " Time Allowed for Completion". The revised estimated contract completion date is October 15th, 2003.
