All copies forwarded to Consultant for signature shall be returned to the City of Gilroy properly filled out. Upon acceptance by the City, the Consultant's copy will be returned to him as his authori to proceed with the work. This amendment to Blair, Church & Flynn Consulting Engineers is for additional services, which includes: providing Utility Policy Certification, Utility Matrix, Traffic Control Plan, and incorporating pothole information performed along First Street provided by HydroScience Engineers in order to obtain a Caltrans Encroachment Permit for an amount not to exceed $18,000, pursuant to the attached May 23, 2017 proposal letter from Kristian Calma to Christine Salmo. Amendment extends the term of the Agreement for Services between the City of Gilroy and Blair Church and Flynn Consulting Engineers, dated 08/31/2017 to 08/31/2020. All requirements of the original Agreement Documents shall apply to the above work except as specifically modified by this Amendment. The contract time shall not extend unless expressly provided for in this Amendment. Blair Church and Flynn Consulting Engineers hereby agrees to perform the above work subject to the terms of this Amendment for obtaining Caltrans Encroachment Permit and for incorporating pothole results. Project Understanding The City of Gilroy plans to rehabilitate approximately 3,000 feet of the existing sewer main in First Street, generally extending from approximately 170 feet west of Wayland Lane easterly and downstream to Monterey Road. Plans and specifications for these improvements were originally submitted to the City of Gilroy on July 22, 2016. As First Street is also State Route 152, the existing right -of -way is a Caltrans right -of -way and therefore will require a Caltrans Encroachment Permit. Based on e-mail communications between yourself and Blair, Church, and Flynn, a Caltrans Encroachment Permit Application is to be completed for this project and recent pothole information along First Street has been obtained by HydroScience Engineers and is to be implemented into the previously submitted plans for this project. A previously submitted encroachment permit application for a nearby project and comments were provided for reference in creation of this new application for the proposed project in First Street. Based on the provided documentation, a Utility Policy Certification, along with a completed Utility Matrix, Traffic Control Plans, and completed application will be prepared by Blair, Church & Flynn and submitted to the City of Gilroy. The City will sign the permit application and submit the application package to Caltrans, and will pay for any necessary permit application fees. Scope of Services The following Scope of Services outline describes the services we propose to provide to the City of Gilroy for the project. I. Pre - Design Data Collection and Incorporation Phase A. Communicate with HydroScience Engineers regarding recently obtained pothole information along First Street B. Submit Permit Application Package to City for Review III. Construction Document Phase A. Obtain and incorporate Caltrans Encroachment Permit review comments B. Prepare and submit final plans to the City for approval Services Outside of Scope Services not included under our scope of work, but that may be provided upon request, include the following: 1. Topographic surveys Proposed Compensation Blair, Church & Flynn proposes to provide the engineering services described above for compensation on a lump sum basis as shown in the following table.
